About the Organisation

Nottingham Forest Foundation is the official charity of Nottingham Forest Football Club. Using the power of Nottingham Forest and football, the Foundation creates positive opportunities and lasting change for people and communities across Nottingham and Nottinghamshire. Its work is shaped around three strategic pillars — Place, People and Potential — supporting healthier, more connected communities and helping people fulfil their potential.

About the Role

The Health & Wellbeing Coordinator will be responsible for the coordination, delivery, development, monitoring and evaluation of Nottingham Forest Foundation's Health & Wellbeing programmes. Working closely with the Health & Wellbeing Manager, practitioners, assistants and external partners, the role-holder will lead the successful delivery of community-based physical activity, health improvement, mental wellbeing and social inclusion programmes across Nottinghamshire.

The role combines programme coordination, community engagement, partnership development and frontline delivery, supporting the Foundation's mission of improving physical health, mental wellbeing, social connectedness and healthier lifestyles within communities experiencing the greatest need. The Coordinator will oversee key programmes including Healthy Goals and Resilient Reds — both supported and administered through the Premier League Foundation & PFA's Community Fund — while also supporting other initiatives such as Walking Football, Forest Forces and Tricky to Talk.

The role-holder will work with a wide range of participants, including teenagers, adults, older adults and individuals facing health inequalities, helping them access support, set personal goals, increase physical activity levels, improve mental wellbeing and build meaningful social connections. The Coordinator will also be responsible for programme compliance, quality assurance, stakeholder engagement, staff support, monitoring and evaluation, and contributing to the development and sustainability of the Foundation's Health & Wellbeing offer.

What You'll Do

Lead on-the-ground delivery of both the Resilient Reds and Healthy Goals programmes, enabling a blend of physical and mental wellbeing activity, interventions, trips and events for communities across Nottingham City and County
Engage in proactive partner and stakeholder engagement, including NHS and Public Health sector organisations, to meet participant need and strengthen referral pathways into Foundation-led health and wellbeing programmes
Work with the Health & Wellbeing Manager to connect and engage programmes with statutory and non-statutory mental health support services (such as Wolf Pack Project and NHS Talking Therapies), developing opportunities for partnership and referral
Coordinate and oversee Healthy Goals physical activity sessions, ensuring venues in high-priority areas host interventions such as physical activity, mental wellbeing and healthy lifestyles guidance
Support the coordination and promotion of Resilient Reds Hubs, using community spaces to enable the reach and engagement of priority audiences
Proactively liaise with internal stakeholders including Finance, to ensure budgetary planning and alignment of programme operations, and Marketing, to communicate programme success and grow participation and engagement
Proactively connect and engage with statutory and non-statutory third-sector service providers, developing partnership opportunities and collaboration with Hubs and programmes where there is latent demand for additional provision
Work continuously with internal and external stakeholders to grow the programme, improve its outcomes, and attain its KPIs
Lead efforts to secure ongoing funding for the health and wellbeing programme, including identifying opportunities, supporting bid writing and cultivating relationships with funders
Collate and organise participant data through qualitative and quantitative measures, such as registers, surveys, case studies and individual testimony from PLF and PFA-funded programmes
Collate and maintain up-to-date programme data across monitoring systems, aligning with PLF and PFA parameters and ensuring all reporting deadlines are met
Act as a positive role model and representative of the Foundation, upholding the same standards expected of other health and wellbeing delivery staff
Support other Health & Wellbeing programmes and wider Foundation delivery where appropriate, incorporating unique skills, knowledge and experience
Uphold all Foundation policies, maintain professional boundaries, and be committed to the highest safeguarding standards, reporting concerns in accordance with procedure
