Thr1ve Social Prescribing (Chesterfield FC Community Trust)
ABOUT THIS PROGRAMME
Chesterfield FC Community Trust Thr1ve Social Prescribing is an award-winning service whose specialist link workers support young people aged 14-18 and young adults aged 19-25 (no diagnosis needed) through 1:1 support and group sessions. The short-term, non-clinical service connects young people with their communities to improve their mental health and wellbeing, helping them access services, activities, education, training and voluntary opportunities. Referrals cover young people registered with North Hardwick and Bolsover PCN and North Derbyshire PCN GP practices, plus those living in Chesterfield, Glossop, Buxton, North-East Derbyshire, Bolsover, Amber Valley and the Derbyshire Dales, prioritising vulnerable groups including care leavers and young people not in education, employment or training.
